All participants must complete and sign the appropriate waivers. Digital acknowledgment or printed signatures are accepted.

Important Reminder
No participant will be allowed to participate in any Utah Elite Hockey event without a completed and signed waiver. This is a firm policy for the safety and legal protection of all involved.
